Section A: General Details
Q1.Name of Applicant
aIf you are a sole trader, eg, John Murphy trading as “Murphy Alarms “ , then the Name of the Applicant is your own name, ie John Murphy.
bIf the business is a partnership, eg., John Murphy and Sean Ryan trading as “ M&R Alarms “, then the name of the Applicant is “ M & R Alarms “.
cIf the company is a limited company, eg “Murphy Alarms Ltd” then this is the Name of the Applicant
dIf the business is a division of a larger company, eg “Spark Alarms”, a division of Spark Electrical Contracting Ltd, then the Name of the Applicant is Spark Electrical Contracting Ltd, since a division is not a legal entity for these purposes.
Q2Business Name: The name by which the business is generally known, eg. “ Murphy Alarms” If the business is a limited company and trades under its own name there is no need to fill in this section .
Q3Business Telephone & Fax numbers – self –explanatory.
Q4Web site address and email address – if you have one.
Q5If you are a sole trader you fill in your PPS ‘social security’ number here. If the business is a limited company or partnership it will have a registered number with Revenue; this is the number they are looking for here.
Q6The address from which the company normally trades. If the company has branches, this would be the Head Office address.
Q7Number of employees, not counting yourself as the owner, or the Directors if the business is a limited company.
      
Section A: Legal Status
Q8Legal Status
aSole trader – if you do business on your own account, with no partners in the business (not even your wife) and you are not a limited company.
bPartnership – if you share control of the business with a family member (wife, brother, etc) or a friend or acquaintance.
cPrivate Unlimited company – a rare animal, normally reserved for charitable organisations, Associations or other not-for-profit organisations.
dPrivate limited company – the most common type, eg, Murphy Alarms Ltd
ePublic Limited Company – multi-national or other large company whose shares are traded on the Stock Exchange.
fForeign Company – if your head office is in another country, eg UK, Germany, US, etc.
gOther – any other exotic entity such as Maintenance Dept of a Govt body, certification laboratory, Diplomatic Mission security personnel, etc.
      
Section A: Turnover
Q9Turnover: This section asks about your turnover (annual sales) and the period for which you make up your accounts. Most business people nowadays run their accounts from 1st January to 31st December, in which case box 1 is “ 010108 to 311208 “
      
Section A: Licence(s) Required
Q10Licence Requirements: Here you must work out how much the licence will cost you.

You should fill in your last annual sales figure, fill in the discount which relates to how much of the company’s sales is for non-relevant sales, eg, fire alarm sales, lock sales, or other nonsecurity installation work, add the fee which relates to this level of turnover (see chart on PSA web-site) and fill in the total box.
      
Section A: Method of Payment
Q11Method of Payment:While you may pay by several methods, for record and security purposes we would recommend to send a cheque, crossed “a/c payee only” and made out to the Private Security Authority.
      
Section A: Tax Clearance Certificate
Q12Tax Clearance Certificate: If you don’t have one don’t bother sending this form in. Get one, then fill in the number listed on the certificate and the period for which it is valid.
      
Section A: Previous Business
Q13Previous Business: This section asks if you have been involved as a Director in a business which went into liquidation (bust ) during the past two years.
If a business you were involved in closed down many years ago, this is not relevant.
